- This online form should be completed by an ADULT GROUP LEADER (Parent/Legal Guardian/ Youth Minister/ Lead Chaperone/ etc.) Teens/individual registrations should not fill out this form.
- The email address should pertain to an adult who is registering the teen(s (YOUTH MINISTER\ LEAD CHAPERONE\ PARENT OR LEGAL GUARDIAN)- and who is responsible to share the information shared via email with all participants.

- I understand... REQUIRED I understand that each participant will need to bring a bagged lunch, water bottle(s) and wear closed toe shoes . I understand that it is expected that participants will be present for the entirety of the day. It will NOT be possible to drop off a teen late or pick up early. I understand that a signed permission slip for every participant attending is required. (Please see our website to download permission slip) I understand that each participant is asked to bring a donation item as their "fee" for the day. Please see the list of items requested for your date and location. These items will directly be used as a part of the projects we do that day. Please fill out this field.
